CV5369 cathode ray tube. This is a dual trace oscilloscope tube with a flat face.

 

Size This CRT measures 375x105mm overall and has a B12F base plus 3 sets of side connectors and an EHT connector

NB this data comes from the data sheet for a 4LP1 CRT assuming that this tube is the same except for having P31 (green) phosphor.

The side connectors are as follows (refer to the datasheet for 4LP1): two connectors on the same side as the post deflection accelerator anode (anode 4) connector are anode 3 and Y1; two connectors on the opposite side are anode 3 and Y2, two connectors closer to the screen are X1 and X2.
